Indiana's Premier Skilled Nursing Home Facility

Green House Senior Living L.L.C. is the first nursing home in Indiana to be licensed to construct "small house" homes for providing skilled nursing care. Green House Cottages of Carmel consists of six cottages that are each 8,000 square feet with 12 private bedrooms including private bathrooms and showers. Homes will also include a family-style hearth room with an open kitchen and dining area. While licensed as a skilled nursing facility, the six cottages are designed to look like the single-family homes in the surrounding neighborhood. Homes feature high-vaulted ceilings in the hearth rooms of each cottage, windows and natural light throughout the homes, granite surfaces, and solid wood doors. We are proud to offer a comfortable home atmosphere for the elders to receive care and call home in Carmel, Indiana.

We are Indiana’s first Green House Project facility. The Green House Project model of care specializes in providing skilled nursing care in a facility that is designed like a home. Elders can reach beyond the status quo in a Green House Project facility and are assisted and encouraged to thrive and grow despite their need for skilled nursing care.

Caregivers who live and work in each of the cottages collaborate with the elders to create a lifestyle that meets the specific needs of each individual. There is no predetermined routine; the goal is to facilitate elders’ independence and ability to pursue their interests.

Each home is staffed with a small, self-managed, team of Certified Nursing Assistants (CNAs). Green House CNAs are trained in household management. While these care partners provide around-the-clock personal care and manage the home, a clinical support team of nurses is available 24/7. In addition, social workers, therapists, physicians, activity coordinators, and dietary professionals are available on an as-needed basis. Learn more about the surrounding Carmel community.

Our Leadership Team

EXECUTIVE DIRECTOR: MELODY DECOLLO, NHA

Melody DeCollo is the Executive Director of Green House Cottages of Carmel L.L.C. With more than fourteen years in healthcare, first as a social worker and then as nursing home administrator, Melody has experienced the healthcare industry at large, national corporations and smaller, local organizations.

Melody’s career in healthcare has equipped her with hands-on knowledge of operations management; financial review, control, and coordination; revenue generation enhancements; budget development, monitor, and evaluation; and oversight of quality assurance programming. In 2010, Melody received the “Community of the Year” award and a “Deficiency Free” facility assessment.

She is a graduate of Southern Illinois University-Carbondale with a B.S. in Social Work and M.S. in Rehabilitation Administration and Services. She is currently licensed in California, Illinois and Indiana as a Healthcare Administrator. When she is not working, Melody enjoys spending time with her family and friends. She loves pilates, yoga, and watching sports.

Admissions and Marketing Director: Gina Middaugh

Gina Middaugh is the Admissions and Marketing Director of Green House Cottages of Carmel L.L.C. She comes to us with 20 years of health care experience working in several different realms of care such as long term care, acute hospital care and home health care. Gina earned her Bachelor of Science degree from Indiana University. She then began her career in long term care. Her experience spans companies both large and small as well as non-for-profits. 

Gina’s passion is helping families navigate through the health care system. She thrives on being a resource for elders and their families. Her experience has allowed her to serve as a board member for the Boone County Senior Services for the past 3 years.

She resides in Carmel with her husband and children. Gina is an active member of Trader’s Point Christian Church.  She enjoys the outdoors, spending time with her family and watching her son play soccer.
